Driver Services Forms and Other Documents

Most Motor Vehicle forms must be printed, filled out manually and delivered or mailed to the address indicated on the form. Forms cannot be submitted online. Please follow any instructions carefully. See Help for assistance with viewing Portable Document Format (PDF) files.

Fill/Save forms – Most forms contain form fields that can be filled in on-screen. You may fill the form in on your computer, save and print the completed form. To save the typed in text in the PDF form, you must have Adobe Reader 7.03 or later. Be sure to sign it before delivering or mailing it to the address indicated on the form.

Application for Military Registration For Montana residents who entered the service from Montana, are on active military duty and are stationed outside the state. Mail renewal forms for regular Class D driver license –For individuals in counties without a driver exam station to renew a regular driver license by mail. Read Renewal by Mail instructions before filling out renewal forms. All three of the following forms are required.

Mail renewal form for out-of-state mail-in license – This is an out-of-state mail-in renewal form for Class D and Commercial driver licenses. There are two conditions to using this form: the applicant must be a Montana resident who is temporarily out of the state; and the applicant cannot use this renewal method consecutively (i.e., the next time they renew, it has to be in person). Read Renewal by Mail instructions before filling out renewal forms.
